Peace of Mind. USA Volleyball Certificates  of Insurance (C of I) is a member benefit to all USA Volleyball clubs and tournament directors who sanction their practices, tryouts, tournaments, and more with North Country Region - USA Volleyball.  Because USA Volleyball Secondary Liability Insurance remains a leader in providing peace of mind to sports organizations, specifically youth sports, USA Volleyball does require the following for the insurance to be valid: 

• Current USA Volleyball membership for all participants
• Adult members who work with minor age athletes at the event meet USA Volleyball safety requirement
• Club director/tournament director/club administrator is able to validate that USA Volleyball CofI requirements were met

WHO COMPLETES THIS FORM?   Club director or  designated club administrator who meets the following requirements:

• Membership – must have current NCR – USA Volleyball membership
• SafeSport training – must be current, cannot expire prior to or during date of event
• Background Screen – must be current, cannot expire prior to or during date of event

NEXT STEPS

• Complete request
• Start date CANNOT be post dated. Start date can be day C of I is created or later.
• Allow 7 – 10 business days for request to be completed
• Certificate of Insurance will be emailed to the email address provided in the request form

September 1, 2023 is the earliest date that a Certificate of Insurance can be issued.
